Moms In Motion - A Virginia Service Facilitation Provider
Moms In Motion is seeking a Service Facilitator (SF) Case Manager in Virginia. You’ll travel locally to client homes, connect families with Medicaid Waiver programs (CCC+, CL & FIS, EPSDT), and help them live safe, healthy, independent lives. If you’re compassionate, organized, and love building strong community ties, you’ll fit right in. No degree required; 2+ years helping individuals with disabilities or the elderly preferred, and a valid driver’s license is a must. #J-18808-Ljbffr
