Moms In Motion - A Virginia Service Facilitation Provider
Moms In Motion in Virginia is seeking a Service Facilitator (SF) Case Manager who will visit families in their homes, helping them navigate Virginia's Medicaid Waiver programs and coordinate care.
You will write plans of care, assess needs, and troubleshoot authorizations, timesheets, and related tasks while building trusting relationships. Remote work from a laptop is supported, with local travel required.
