Moms In Motion - A Virginia Service Facilitation Provider
Moms In Motion is seeking a Service Facilitator (SF) Case Manager who will live in Virginia and travel locally to client homes. You will guide families through Virginia’s Medicaid Waiver programs (CCC+, CL & FIS, EPSDT) and help clients live safe, healthy, independent lives. Ideal candidates have 2+ years of experience in disability or elderly support, strong communication skills, and a tech-savvy mindset.
