Community Outreach & Care Coordinator - Substance Use
Evergreen Healthcare Partners
Evergreen Healthcare Partners, LLC is seeking a Service Coordinator for StepOne Service, a hospital-based Acute Withdrawal Management program for adults. The role works with hospital leadership and StepOne Service leadership to broaden community visibility and coordinate care for patients with substance use disorders.
The position requires knowledge of substance use disorders, social services experience, and HIPAA familiarity, with 2+ years in related roles.
