Our innovative Medical Rehabilitation Program offers a meaningful alternative to institutional rehab, helping people heal at home, with dignity and purpose.We are proud to have ranked in Boston Globe's Top Places to Work in Massachusetts and USA Today's Best Places to Work in the United States since 2023!Position Details:Main Office location: Concord, MAService Area: Concord Center TerritoryJob Type: Full TimeSchedule: Monday - FridaySalary Range: $100,000.00 - $115,000.00Responsibilities:Lead and support the Center's clinical team, providing day-to-day guidance, coaching, and problem-solving to help clinicians deliver exceptional patient care.Oversee daily clinical operations, including new admissions, patient assignments, visit completion, staffing needs, and continuity of care.Lead daily clinical meetings to keep the team connected, address patient needs, identify barriers, and coordinate priorities. (In person once per week and virtual the other four days)Serve as a clinical resource for staff, assisting with complex patient situations, care planning, documentation, physician communication, and other clinical needs.Monitor patient care and clinical documentation to promote quality, regulatory compliance, timely billing, and positive patient outcomes.Manage and develop team members through regular feedback, joint visits, case reviews, competency assessments, performance evaluations, recognition, and ongoing education.Partner with Recruiting and leadership to identify staffing needs and participate in interviewing, selecting, onboarding, and retaining clinical team members.Monitor clinician schedules, productivity, and caseloads while balancing patient needs, staff capacity, and appropriate use of per diem resources.Support quality and safety initiatives, including incident review, infection control, patient feedback, staff education, and performance improvement efforts.Collaborate with Administrative, Marketing, and other clinical leaders to support referrals, census growth, efficient operations, and a positive patient and employee experience.Maintain clinical competency and provide patient or joint visits when needed to support patients and team members.Qualifications:This position requires candidates to hold an active RN, PT, or OT license in the state of Massachusetts.Maintain current BLS CPR certification.Minimum of two (2) years of direct clinical experience; home health preferred.Prior supervisory experience preferred.Excellent written and verbal communication skills.Proficient with computers, tablets, smartphones, EMR systems, email, and other electronic systems.Ability to foster a positive and purposeful culture.Our Benefits:Medical Coverage: Choice of HMO and PPO medical plans to meet the needs of you and your familyDental & Vision: Comprehensive dental and vision coverageLife Insurance: Employer-provided life insurance for added peace of mindPaid Time Off:3 weeks of PTO annually, accrued throughout the year4 weeks of PTO annually accrued throughout the year, after two years of serviceSick Time: 1 week (40 hours) of paid sick time per yearShort-Term Disability: 100% company-paid short-term disability coverageFlexible Spending Accounts (FSA): Pre-tax accounts available for healthcare and dependent care expenses401(k) Retirement Plan with employer matchEmployee Assistance & Benefits Support through BennieVoluntary Benefits including supplemental life, accident, and disability insurance8 Paid Holidays observed annuallyProfessional Development & Training OpportunitiesPathways Healthcare is committed to providing exceptional care to our patients and fostering a positive work environment for our team members.
