Responsibilities
- Collect data on residents’ physical and psychosocial status and communicate findings to the RN and Interdisciplinary Team.
- Contribute to a developmentally appropriate, culturally relevant plan of care reflecting nursing diagnoses and interventions; involve resident/family and care team to achieve outcomes.
- Propose revisions to the plan of care after evaluating resident outcomes; identify referrals to address all aspects of care.
- Provide direct and indirect skilled nursing care under the supervision of an RN; maintain ongoing clinical knowledge and participate in in-service programs.
- Collaborate with residents, families, physicians, and other professionals; support a safe, patient-centered environment and ensure documentation and safety standards are followed.
- Mentor and orient junior staff when applicable; apply safety, cost, and resource considerations in care delivery.

Requirements
- Active NY Licensed Practical Nurse license
- BLS certification (American Heart Association or RQI)
- Minimum 1 year of LPN experience in a long-term care setting
- Ability to provide care to adult and geriatric residents
- Availability to work Day/Evening/Night rotations as needed
- Ability to work in the listed travel assignment setting
