Overview
Licensed Practical Nurse (Long Term Care) travel assignment in Belen, New Mexico. Start date: 09/10/2026 for 15 weeks. Weekly pay: $729-$780 per week. Guaranteed hours: 36 hours per week. Shift: Nights, 3x12-hour shifts (18:00–06:30). Setting: Skilled Nursing Facility providing long-term care. Travel temporary assignment through Healthforce Healthcare LLC.

Responsibilities
- Administer prescribed medications and document administration accurately in the patient chart.
- Monitor vital signs, identify deviations, and report changes per policy; respond to emergencies as needed.
- Provide routine wound care and dressing changes; support infection control practices.
- Assist residents with activities of daily living and mobility; support safe transfers and positioning.
- Collaborate with the interdisciplinary team; document care notes in the electronic medical record.
- Support fall prevention, patient safety, and adherence to care plans during night-shift coverage.

Requirements
- Active LPN license in New Mexico
- Completion of an approved practical nursing program (LPN)
- CPR/BLS certification
- Experience in long-term care or skilled nursing facility settings preferred
- Ability to work nights; available for 3x12-hour night shifts
- Strong communication and documentation skills; ability to work within a team
