Specialty Case Manager

Community Bridges, Inc. (CBI) is an integrated behavioral healthcare agency offering a variety of programs across Arizona, including residential, outpatient, inpatient, patient-centered medical homes, medication-assisted treatment, and crisis services for individuals experiencing crisis, opioid use disorder, homelessness, and mental illness. Specialty Case Manager This role provides case management services in the community to individuals with mental health and/or substance use diagnoses, survivors of domestic violence, and high-risk clients. Responsibilities include planning, connecting, advocating, coordinating, and monitoring services to help patients access health care, financial assistance, housing, employment, education, social services, and natural supports. The case manager assesses needs, develops and monitors individualized treatment plans, and documents services accurately and promptly. Responsibilities Plan, connect, advocate, coordinate, and monitor services for patients with mental health/substance use diagnoses, domestic violence survivors, and high-risk clients. Assist patients in accessing health services, financial assistance, housing, employment, education, social services, and natural supports. Assess client needs and develop individualized treatment plans. Document services and ensure accurate, timely record-keeping. Implement advanced techniques and interventions tailored to high-risk individuals. Qualifications High school diploma or GED required; associate’s degree or higher in a field related to behavioral health preferred. Recovery from alcohol or drugs and/or general mental health for equal to or greater than one consecutive year preferred. 1‑3 years of experience in a behavioral health position providing outreach and engagement activities preferred. 1‑3 years of experience working with special populations (e.g., serious mental illness, domestic violence survivors, high-cost/high-need clients) preferred. Or equivalent combination of education and experience. Current Arizona driver’s license, valid and in good standing, and 39‑month MVR. Peer certification (if applicable).
