Work Environment\n

This remote position allows you to maintain flexibility while working closely with a dynamic, interdisciplinary team within a hospice center. You will collaborate with healthcare professionals, including physicians, nurses, and other therapists, to deliver invaluable support to patients and their families. The hours are adaptable, with options for either 4x10 or 5x8 shifts, created to empower our employees to maintain a healthy work-life balance.

Key Responsibilities\n

• Deliver individualized psychotherapy and counseling to patients and their families, addressing issues like grief, loss, and chronic illness.

• Conduct thorough interviews to gather relevant social, psychological, and health information required for effective treatment planning.

• Evaluate clients' emotional and psychological well-being, providing assessment and treatment within the scope of social work practice.

• Design and implement programs and activities that meet the social and emotional needs of clients, particularly in a hospice setting.

• Facilitate cognitive and behavioral therapeutic interventions, addressing crises and providing community resource referrals when needed.

• Foster a therapeutic relationship founded on trust, empathy, and respect to enhance client engagement and treatment outcomes.

• Continuously assess and document the effectiveness of therapeutic interventions and adjust care plans according to client progress and feedback.

• Maintain accurate and timely documentation in client records, ensuring compliance with all established protocols.

• Manage a caseload effectively, keeping client documentation up-to-date and reflective of the services delivered.

• Participate in team meetings and training sessions to stay informed about the latest therapeutic practices and hospice care protocols.

Qualifications\n

• A Master's degree in Social Work, Counseling, Psychology, or a related field is required.

• A current, active license as a Clinical Social Worker (LCSW) in Tennessee is mandatory.

• A minimum of four years of relevant clinical experience, demonstrating proficiency in mental health counseling, particularly concerning behavioral health issues.
