Registered Nurse – Home Health

Schedule & Territory

Monday–Friday: 8:00 AM–5:00 PM
No weekends or on-call
5 visits per day
Mileage reimbursement: $0.43/mile
Coverage: Fond du Lac, Dodge, Columbia & Winnebago Counties
Additional locations: Brookfield, Jackson, Waterford & Sheboygan

Position Overview

We are seeking an experienced Home Health RN to provide skilled nursing care, patient assessments, care coordination, and disease prevention services in the home setting.

Responsibilities

Assess patients and develop, implement, and evaluate individualized care plans.
Provide skilled nursing care, including wound care and medication management.
Coordinate care with physicians, patients, families, and other healthcare professionals.
Complete accurate clinical documentation and OASIS assessments.
Monitor patient conditions and communicate changes to the appropriate providers.
Support patient education, health maintenance, and disease prevention.

Qualifications

Active Wisconsin RN license required
Home health experience required
Wound care experience required
OASIS experience required
HCHB experience highly preferred ; MatrixCare or other EMR experience accepted
Current CPR/BLS certification
Valid driver's license, reliable vehicle, and current auto insurance

HCHB or hospice experience is strongly preferred. Candidates without HCHB or hospice experience may be required to complete additional training.
