Expected start date: ASAP / Ideally 11/23/2026 Provider Type MD Specialty: Family Medicine FTE needed: 1 Daily Shift Standard: 8hr Expected duration of assignment Current approval end date is 5/6/2027 Reason for opening: Gap in coverage

Schedule /Shift hours M-F 8a-5p Type of privileges: clinic/hospital Setting: Outpatient Call requirement: No Rounding, admissions, or none? None Patient demographics All Ages Average patients per day: 16-18

Required Procedures None Required. Desired include skin excisions/punch biopsies, Nexplanon & IUD, Cryotherapy, toenail evulsions. Required clinical activity in the last 24 months (case logs) Applicant must demonstrate involvement as an admitting physician for at least (20) patients during the past two (2) years. Will provider work solo? Yes Licensed Candidate: No, but highly preferred with start date in 12/2026 Required certifications (Must be from American Heart Association or American Red Cross as applicable) BLS EMR: EPIC
