Mental Health Case Manager
Centers-For-Independence
Centers-For-Independence (CFI) in Milwaukee, WI is seeking a Case Manager to provide treatment and care for mentally ill adults. You will develop and implement treatment plans, monitor medications, and coordinate with community resources to help clients achieve housing, income, and wellness goals. This role requires a Bachelor's degree in Social Work or related field, at least 1 year of experience, strong English (Spanish helpful) communication, proficiency with Word and Excel, and a valid #J-18808-Ljbffr
