Work Environment

This position requires working onsite at our home care facility in Oxnard. The work environment is dynamic and supportive, featuring robust resources and a team-oriented culture. Key Responsibilities

• Conduct thorough assessments of patients' physical health and create individualized treatment plans that take into account their unique challenges and goals.
• Utilize evidence-based physical therapy methods, including therapeutic exercises, manual therapy, and patient education, to address specific physical health conditions.
• Monitor patients' progress toward their therapeutic goals, adjusting treatment strategies as necessary based on ongoing assessments.
• Collaborate with multidisciplinary team members to ensure cohesive and comprehensive patient care, sharing insights and strategies to enhance treatment outcomes.
• Educate patients and their families about physical health conditions, treatment protocols, and self-management strategies to empower them in their recovery journey.
• Document all patient interactions and maintain confidential records in compliance with legal and ethical standards.
• Participate in ongoing training sessions, case consultations, and professional development activities to stay informed about advancements in physical therapy.
• Engage with patients in crisis situations by mobilizing appropriate resources and support systems, ensuring a safe and compassionate environment.

Patient Population / Client Focus

Our physical health therapy services are designed to assist individuals with a range of conditions, including but not limited to, musculoskeletal injuries, chronic pain, post-surgical rehabilitation, and mobility issues. Our diverse patient population includes adults and seniors, many of whom may also contend with co-occurring mental health conditions. Your role will be crucial in not only addressing physical ailments but also promoting overall wellness.

Qualifications

• Valid Bachelor's Degree in Physical Therapy from an accredited institution.
• Current and valid state licensure as a Physical Therapist in California.
• Proven knowledge and practical experience in physical therapy techniques and patient care.
• Strong understanding of physical health assessments and intervention strategies tailored to diverse patient needs.
• Ability to establish rapport with patients, demonstrating empathy and effective communication skills.
• Strong organizational skills and attention to detail in maintaining accurate patient records.
• Experience in using electronic health record (EHR) systems is a plus.

Preferred Experience

• Experience in a behavioral health setting or with clients facing mental health challenges.
• Familiarity with interprofessional collaboration principles and an understanding of how physical health influences mental well-being.
• Continuing education and specialization in specific therapeutic modalities are preferred.

Compensation and Benefits

The salary for this position ranges from USD 115,000 to 130,000 per year, commensurate with experience and qualifications.
