Case Management Social Worker - Pediatrics Hematology/Oncology

As a Case Management Social Worker in our Pediatric Hematology & Oncology department, you will play a vital role in supporting children and adolescents diagnosed with cancer and blood disorders, as well as their families, throughout the continuum of care. You will coordinate comprehensive care and services through psychosocial assessments, emotional support, crisis intervention, and resource navigation tailored to the unique needs of pediatric hematology and oncology patients. In this role, you will provide supportive counseling to patients and families coping with complex medical diagnoses, treatment‑related challenges, and psychosocial stressors. You will collaborate closely with an interdisciplinary team to address barriers to care, facilitate discharge planning, connect families with community and financial resources, and ensure safe, effective transitions across care settings. You will also advocate for high‑risk patients and families, helping them navigate the healthcare system while promoting resilience, well‑being, and optimal treatment outcomes. A caregiver in this role works Monday – Friday from 8:00 a.m. – 4:30 p.m. with virtual on‑call requirements. A caregiver who excels in this role will: Coordinate the care and services of select patients. Intervene with patients who have complex psychosocial and discharge planning needs. Conduct detailed psychosocial assessments and coordinate with community resources and supportive counseling. Offer crisis interventions to patients and families with psychosocial needs and facilitate the transition of care for high‑risk patients facing barriers to care. Manage all aspects of discharge planning for assigned patients in order to ensure a timely discharge and provide appropriate connection with post‑discharge care providers. Work with families exhibiting complex family dynamics. Communicate with interdisciplinary team regarding the discharge planning status of all patients that are referred. Educate patients, families and physicians regarding post‑acute options and address freedom of choice and financial concerns. Conduct initial and ongoing psychosocial assessment and interventions and make treatment recommendations to engage patients and families in adherence to the treatment plan by reducing or eliminating the social, psychological, financial, behavioral and regulatory barriers to successful medical outcomes. Serve as a resource person to provide counseling and intervention related to treatment decisions and end‑of‑life issues. Advocate for patients and families by providing intervention in cases involving child abuse and neglect, domestic violence, elderly abuse, institutional abuse and sexual assault. Advocate for patient and family empowerment and independence to make autonomous health‑care decisions and access needed services within the health care system and community. Meet core social work competencies and adhere to regulations. Remain current with regulations which impact the care of patients and regulatory compliance. Promote individual, professional growth and development and meet mandatory and continuing education requirements. Serve as a preceptor, mentor, and resource for less experienced caregivers and represent the department on committees. Minimum qualifications for the ideal future caregiver include: Master’s degree in social work (MSW) or Master of Science in Social Administration (MSSA) Current state licensure as a Social Worker (LSW, LISW or LISW‑S) Basic Life Support (BLS) certification through the American Heart Association (AHA) or American Red Cross Preferred qualifications for the ideal future caregiver include: Certification as a Case Manager within one year of eligibility from an approved professional organization Experience working with children and families Three years of experience in a healthcare system that includes exposure to and experience in pre‑ and post‑acute care Care Management/ Hospital Social Work experience Knowledge of community resources Epic experience Physical Requirements: Requires full range of motion, manual and finger dexterity and eye‑hand coordination. Requires standing and walking for extensive periods of time. Requires normal or corrected hearing and vision to normal range. Sedentary work exerting up to 10 pounds of force occasionally (Occasionally: active or condition exists up to 1/3 of the time) and/or a negligible amount of force frequently (Frequently: activity or condition exists from 1/3 to 2/3 of the time) to lift, carry, push, pull, or otherwise move objects, including the human body. Sedentary work involves sitting most of the time. Jobs are sedentary if walking and standing are required only occasionally and all other sedentary criteria are met. Pay Range Minimum Annual Salary: $57,510.00 Maximum Annual Salary: $87,697.50 The pay range displayed on this job posting reflects the anticipated range for new hires. A successful candidate’s actual compensation will be determined after taking factors into consideration such as the candidate’s work history, experience, skill set and education.
