LTSS Service Coordinator - Clinician (LSW, LCSW, LPN)

LTSS Service Coordinator – Clinician (LSW, LCSW, LPN) Field‑based role across Ohio, working in clients’ homes or designated sites with occasional office meetings. The role focuses on trauma‑informed, culturally competent, person‑centered coordination for members’ physical, behavioral, long‑term services and psychosocial needs. Responsibilities Assist in identifying high‑risk members and conduct telephonic or face‑to‑face assessments. Collect and provide clinical data to the responsible RN for verification and interpretation. Support the RN in developing, monitoring, and revising members’ care plans. Coordinate care for members with chronic illnesses, comorbidities, or disabilities, ensuring cost‑effective utilization of health benefits. Make decisions based on current member needs and clinical guidelines without requiring nursing judgment. Minimum Requirements Current, unrestricted license: LPN/LVN, LSW, LCSW, LMSW, or other professional license authorized under state law (non‑RN). Nursing Diploma or AS in Nursing or related field. Minimum2years of experience working with individuals with chronic illnesses, comorbidities, or disabilities in a Service Coordinator or similar role, or equivalent education/experience. Compliance with applicable state law regarding licensure. Preferred Skills, Capabilities, and Experiences Case management experience with older adults or individuals with disabilities. Bachelor’s degree in Health, Nursing, or related field.
