We're looking for a compassionate, skilled LPN to join our Hospice team in a full-time, field-based role serving patients and families throughout the Cleveland / Northeast Ohio area.
Compensation
- $30.00-45.00 per hour
- Expected 40 hours per week
- Additional coverage opportunities available in Home Health and Palliative Care

What You'll Do
- Provide direct patient care to hospice patients in their homes in accordance with the established plan of care
- Monitor and document patient symptoms, comfort levels, and changes in condition
- Administer medications and treatments as directed by the RN Case Manager or physician
- Communicate effectively with patients, families, and the interdisciplinary care team regarding patient status and care goals
- Provide education and emotional support to patients and families navigating end-of-life care
- Maintain accurate and timely documentation in the EMR within required timeframes
- Provide coverage in Home Health and Palliative Care settings as needed
Requirements
- Valid LPN license in good standing with relevant state licensing boards.
- Proven experience in hospice or palliative care settings is highly preferred.
- Familiarity with EMR/EHR systems such as HCHB
- Knowledge of medical terminology related to geriatrics, dementia care (including Alzheimer's), infectious disease care, and pain management.
- Experience working with individuals with developmental disabilities or in assisted living environments is a plus.
- Strong assessment skills combined with physiology knowledge to evaluate patient needs effectively.
- Excellent communication skills for triage situations and family education; basic life support (BLS) certification is necessary.
