Requirements

• Required certification: Pharmacist
• 1+ years of experience
• Key Requirements Summary: 1. Experience: 1 year required 2. Cases: Inpatient pharmacy experience preferred 3. Certifications: Chemo/Hazardous Drug, Oregon Pharmacist License required 4. License: Oregon Pharmacist License required, can submit with pending unless specified in job title 5. Education: Not specified 6. EMR: Epic experience required 7. Shifts/Call: On-call required for overnight hours; weekends required 0730-1700; block schedules required 8. RTO: 6 days total or 3 consecutive days auto-approved; must commit to working 3 of 5 holidays; all RTO must be requested upfront during submittal 9. Returning Staff: 3 months between permanent and travel; must disclose previous Providence facility employment 10. Travel/Local: 50-mile radius rule; locals accepted at lower rate; must have vehicle for assignment 11. Background: Any background hits require approval prior to submission 12. Modules: Non-billable, average 16 hours; completed pre-start and annually; over 20 hours requires PM approval 13. Travel Experience: Not specified as required 14.
