Registered Nurse - RN Case Manager

About the Role Make a difference in the lives of former nuclear weapons workers by providing in‑home nursing and case management services. You will work on a flexible schedule and earn $44 per hour, including full pay for drive time. Key Responsibilities Conduct weekly in‑home nursing visits (1–2 hours) for clients with chronic or terminal illnesses. Perform physical assessments, including vitals, and ensure home safety with medical equipment and supplies. Fill weekly medication planners and oversee skilled and non‑skilled caregivers. Provide direct care and case management for assigned clients. Collaborate with the regional director and other personnel to identify and correct issues and improve services. Plan, implement, and evaluate care; coordinate client care conferences. Serve as local on‑site clinical resource and support staff scheduling and competencies. Adhere to agency policies, nursing delegation guidelines, and scope of practice. Perform supervisory visits, review documentation, and contribute to performance improvement initiatives. Assist with staffing, training, education, and competency validation. Document and submit data on infections, occurrences, complaints, and grievances. Assist with marketing activities when requested. Qualifications Graduate of a state‑approved school of professional registered nursing (BSN preferred). Current, unrestricted RN license in the state(s) of practice. Minimum of 2 years of nursing experience, including at least 1 year in home care or a closely related field. Minimum of 1 year of supervisory or case‑management experience (preferred). Current CPR certification.
