Facility *National Jewish Health - Main Campus Department Nursing - Adult Clinic Nursing Job Category Registered Nursing Work Setting: Hybrid (2 days/week remote) National Jewish Health is seeking a Case Manager (Registered Nurse/RN) to support patients living with chronic conditions and empower them in their health journeys. This role is an essential link between clinical care and patient self-management, providing the guidance and coordination needed to improve outcomes and enhance quality of life. Position Summary The Coordinated Care Management (CCM) Specialist plays a critical role in supporting patients with chronic conditions by providing education, care coordination, and ongoing case management. This position helps bridge the gap between clinical care and patient self-management by developing personalized care plans, conducting regular follow‑ups, and assisting patients in navigating the healthcare system. Working collaboratively with providers and care teams, the CCM Specialist promotes improved health outcomes, medication adherence, and patient engagement. Responsibilities Develop and update individualized care plans tailored to patient needs and disease acuity. Provide health education focused on prevention, chronic disease management, and lifestyle modification. Conduct regular patient follow‑ups via phone using the ChronicCareIQ (CCIQ) platform. Collaborate with providers and clinical teams to coordinate care and support patient engagement. Promote medication adherence, risk reduction, and improved health outcomes. Assist patients with scheduling and navigating healthcare services while ensuring accurate documentation. Other Duties Other duties as assigned. Minimum Qualifications Education: Graduation from an Accredited School of Nursing required, BSN preferred. Work Experience: Minimum 1 year of experience required. Background in case management or chronic disease care preferred. Special Training, Certification or Licensure: Current Colorado Registered Nurse (RN) license and current CPR/BLS certification required upon hire for all areas.
