Community Health Advocate: In-Home & Outreach
Central Ohio Primary Care
Central Ohio Primary Care Physicians, Inc. is seeking a Community Health Worker to bridge patients with healthcare and social services in Westerville, OH. You will support care coordinators, conduct outreach, and visit clients in homes or offices to promote health and address social determinants of health. This full-time position requires a high school diploma, 3 years of related experience, and a valid Ohio driver’s license.
