Akido Labs is seeking a Community Health Worker to support members with complex medical and social needs in South LA. The role involves outreach, care coordination, and advocating for access to services. You will manage a caseload, build trust with patients, coach on health behaviors, accompany to appointments, and connect to local resources, while documenting encounters in our systems.
