Geriatric Care Social Worker (MSW/BSW) Location: Fairfax, CA
Healthcare / Long-Term Care Setting
Full-Time | Temp-to-Hire
- Conduct psychosocial assessments for geriatric patients
- Support discharge planning and care transitions
- Assist families with long-term care placement decisions
- Provide emotional support and crisis intervention
- Coordinate services with hospitals, SNFs, hospice, and community agencies
- Support patients with dementia, chronic illness, and end-of-life care needs
- Maintain accurate documentation in compliance with facility and state standards
- Collaborate with interdisciplinary healthcare teams
- Bachelor’s degree (BSW or related field) required; MSW preferred
- Experience in geriatric care, healthcare, or long-term care settings
- ASW or LCSW preferred for clinical roles
- Knowledge of California healthcare and aging service systems
- Understanding of mandated reporting requirements for elder abuse
- 2-3+ years in hospital, SNF, hospice, or geriatric care settings
- Experience with dementia/Alzheimer’s populations
- Strong discharge planning and case coordination skills
- Strong communication and family support skills
- Live Scan background check (DOJ/FBI clearance)
- TB test and health clearance (as required by facility)
- Mandated Reporter Training (California elder abuse reporting)
- Valid identification and work authorization
