Key Responsibilities\n

• Conduct thorough, person-centered assessments to understand patients’ psychosocial needs and strengths.

• Develop and implement tailored treatment plans that align with the goals of our patients and their families.

• Facilitate therapeutic interventions, including counseling sessions, support groups, and crisis intervention, to promote emotional healing.
\n
• Maintain precise documentation of patient interactions and treatment outcomes in compliance with HIPAA regulations and organizational protocols.

• Engage actively with the multidisciplinary treatment team, sharing insights and strategies to enhance patient care coordination.

• Maintain up-to-date knowledge of best practices and emerging research to continuously improve the quality of care provided.

Patient Population / Client Focus\n

Our hospice care clientele comprises individuals facing terminal diagnoses, providing an array of challenges encompassing grief, anxiety, and coping with loss. As an LCSW-C, you will play a crucial role in helping both patients and their families navigate these complex emotional landscapes with sensitivity and expertise.

Qualifications\n

• A Master’s degree in Social Work from a Council on Social Work Education (CSWE) accredited institution.

• Current and active LCSW-C license in the state of Maryland.

• Proficiency in clinical assessment and therapeutic methodologies specific to hospice care and palliative services.

• Exceptional interpersonal and communication skills, enabling you to build trust with patients and their families.

Preferred Experience\n

Compensation and Benefits\n
