in Virtual Care Management

Part-time, flexible (10 hours/week).

Position Description:

Ibis Health is the patient-facing affiliate of Senscio Systems, a Harvard, MA based tech-enabled virtual care coordination company with current operations in FL, MA, NH, ME, PA, RI, CT, and NC.  The company’s service delivery includes MDs, RNs and Patient Care Advocates who provide care plan-based virtual services coordination, remote patient monitoring, and home-based self-management coaching to chronic care patients in collaboration with their PCPs and home healthcare providers.

Use of the Ibis Health platform has documented significant reductions in avoidable hospitalizations, improved health outcomes, and overwhelmingly positive patient experience scores, and is recognized for innovating virtual care best practices for polymorbid patients with complex chronic conditions.

The company is currently expanding its operational footprint into the state of Florida and is actively seeking a Florida-licensed Physician Partner to provide general oversight to the dedicated remote virtual care team.

Minimum Qualifications:

• Active FL medical license in Good Standing.
• Approved Medicare Provider Certification.
• Credentialing with regional health plans:

i.e., BCBS (Guidewell/Florida Blue), Aetna, UnitedHealth, CarePlus, WellCare, CIGNA.

• Competency in team leadership, service delivery oversight, and clinical guidance.
• Experience/familiarity in the use of EMRs, digital health platforms and interprofessional communication.
