Job Description

• Assess patients in inpatient or outpatient facilities and craft personalized treatment plans.
• Deliver physical therapy treatments with an emphasis on mobility, strength, and balance.
• Work alongside nurses, doctors, case managers, and other rehabilitation professionals to ensure optimal patient outcomes.
• Inform patients and families regarding therapeutic objectives, planning for discharge, and exercises they can continue at home.
• Maintain detailed documentation of evaluations, treatments, and progress as per facility and regulatory standards.
• Engage in interdisciplinary rounds and care conferences to guarantee seamless care for patients.

Education

• Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited program.

Certifications

• State licensure as a Physical Therapist.
• Preferred: CPR certification.
• Specialty certifications (e.g., in neurology or geriatrics) are advantageous but not mandatory.

Skills

• Prior experience is highly preferred.
