Description

• Conduct thorough evaluations of patients in both inpatient and outpatient settings, crafting customized treatment plans
• Implement therapy interventions targeting mobility, strength, balance, and overall function
• Work collaboratively alongside healthcare providers such as nurses, physicians, and case managers to enhance patient outcomes
• Educate patients and their families regarding therapeutic goals, discharge processes, and tailored home exercise programs
• Maintain accurate documentation of evaluations, treatments, and patient progress in compliance with regulatory requirements
• Engage in team care rounds and interdisciplinary meetings to ensure seamless care delivery

Education

• Doctoral degree in Physical Therapy (DPT) or comparable qualification from an accredited institution

Certifications

• Active state licensure as a Physical Therapist
• CPR certification is preferred
• Specialty certifications (neurological, geriatric, etc.) are advantageous but not mandatory

Skills

• Prior experience is strongly preferred
• Ability to manage patients with intricate medical and mobility challenges
• Exceptional communication and teamwork abilities
• Familiarity with digital charting and clinical documentation systems
