Licensed Clinical Social Worker for Senior Care

Position Overview

The Cognitive Care Center is seeking a Licensed Clinical Social Worker (LCSW) to join our dedicated team in Rockford, Illinois. This full-time remote position offers an opportunity to make a significant impact on the lives of seniors and their families through compassionate support and expert guidance. The LCSW will work closely with clients to assess their psychosocial needs, create tailored care plans, and collaborate with interdisciplinary teams to ensure comprehensive care delivery.

Work Environment

This position is fully remote, allowing for flexibility in work hours while still offering essential support to seniors. The LCSW will engage with clients primarily over virtual platforms but will occasionally conduct in-person assessments and follow-ups when necessary. The work schedule is mainly Monday through Friday, from 8:00 AM to 4:30 PM, with additional evening and weekend hours as required by client needs.

Key Responsibilities

• Conduct comprehensive assessments of seniors' psychosocial needs, focusing on emotional, mental, and social well-being.
• Collaborate with seniors and their families to develop individualized care plans that meet specific goals and preferences.
• Provide counseling and emotional support to clients, assisting them through significant life transitions and challenges.
• Advocate for clients' rights and help them access community resources vital for enhancing their quality of life.
• Work in partnership with an interdisciplinary team, including healthcare professionals such as nurses and caregivers, to coordinate care effectively.
• Perform regular check-ins to evaluate clients' well-being, assess living conditions, and identify potential safety concerns.
• Facilitate communication and mediate conflicts among family members involved in the client's care.
• Educate clients and families regarding available community support, counseling services, and programs that foster social engagement.
• Maintain accurate and timely documentation of all client assessments, care plans, progress notes, and interactions using electronic health record (EHR) systems.
• Stay informed of current social work practices, policies, and ethical standards to ensure compliance in all aspects of care.

Patient Population / Client Focus

The LCSW will primarily serve the senior population within the Rockford area, addressing the diverse psychosocial needs of older adults and their families. Our clients may be experiencing challenges related to aging, such as grief, chronic illness, social isolation, and caregiving stress. The LCSW will be instrumental in supporting this vulnerable demographic, providing the tools and strategies they need to navigate their circumstances successfully.

Qualifications

• A Master’s degree in Social Work from an accredited institution.
• Current licensure as a Licensed Clinical Social Worker (LCSW) in the state of Illinois.
• At least two years of relevant experience in social work, preferably within geriatric or community settings.
• Strong understanding of social work principles and aging-related issues, along with available community resources.
• Excellent interpersonal and communication skills, with the ability to establish rapport with seniors and their families.
• Proficiency in using EHR systems and computer applications for documentation and communication purposes.
• Capacity to empathize with clients, showing compassion and understanding in all interactions.

Preferred Experience

• Experience working in a community center or with a senior population.
• Knowledge of state and federal regulations pertaining to social work and geriatric care.
• Skills in crisis intervention and conflict resolution are advantageous.

Compensation and Benefits

The Cognitive Care Center offers a competitive salary range of USD 60,000 – 100,000 per year, commensurate with experience and qualifications.
