RN Case Manager

Member of the Case Management team. Coordinates and supports healthcare within the facility. Coordinates referrals for services outside the facility. Provides outreach and enrollment services to meet eligibility requirements. Ensures coordination and planning, including community and family support. Focuses on the development and coordination of community service plans. Partners with service providers, families, and patients to create a healthcare plan. Identifies additional service providers as needed. Modifies patient treatment plans based on responses and conditions. Prepares patients and families for discharge. Communicates with third-party payers to obtain necessary authorization for reimbursement. Reviews challenging cases with the medical team as needed. Defines care goals of patients by providing education, information, and direction. Maintains accurate and detailed reports and records. Communicates with third-party payers for necessary authorization of services.
