Job Title: Care Coordinator Address: Lewisville, TX Job Description Applies acquired job skills and company policies and procedures to complete assigned tasks. Works on routine assignments that require basic problem resolution. Receives general direction on standard work and detailed instruction on new assignments. Creates and completes accurate referrals and applications into a centralized customer relationship management platform. Maintains quality while providing an empathetic and supportive experience to the patient. Interacts with patient referral sources to process new applicants. Stewards patient accounts from initial contact through final approval/denial. Critical Information 100% WFH-Remote. Must be able to work any 8hr shift between 7AM-7PM (CST) as assigned. Education/Licenses Needed High School education required. 2-4 years related customer facing experience and/or training desired.
