Housing Navigator: Rapid Rehousing & Community Support
Molina Healthcare of Illinois
Molina Healthcare is seeking a Housing Support Specialist to assist members at risk of homelessness and those recently discharged with housing options, promoting self-sufficiency and community integration. You will identify housing options, assess barriers, and develop action plans to achieve permanent housing while coordinating with community resources.
