FCP Case Manager
Humana
Humana is seeking an FCP Case Manager in Milwaukee, WI to support members in the Family Care Partnership program. In this role, you will conduct comprehensive assessments, develop individualized care plans, and coordinate medical, behavioral, and community services to improve member health and independence. You will manage a caseload, monitor outcomes, and collaborate closely with an interdisciplinary team. Working within Humana’s member-focused, inclusive culture, you’ll use data, clinical insight, and community resources to remove barriers to care and positively impact members’ lives.
