Travel Home Health/Hospice Registered Nurse (RN)

The Travel Home Health/Hospice Registered Nurse (RN) provides skilled nursing care to patients in their homes across the Denver service area. This role focuses on delivering high-quality, patient-centered care while coordinating closely with the interdisciplinary team, physicians, and referral sources. The RN will manage assigned patient visits, participate in daily team communications, and support continuity of care in a collaborative, fast-paced environment.

Schedule & Work Details:

• Location: Denver, CO, United States
• Schedule: Monday - Friday
• Shift Hours: 8:00 AM – 4:30 PM
• Weekly Hour Guarantee: 32 hours
• Hourly Rate: $53.00 – $55.00 per hour
• On-Call / Weekends: One weekend per month
• Mileage Reimbursement: Employee is responsible for travel to and from work. All mileage between patient visits is reimbursed from first patient to last patient of the day

Key Responsibilities:

• Perform assigned home health and hospice patient visits in accordance with plans of care
• Conduct skilled nursing assessments and provide appropriate interventions
• Coordinate care with the office team, physicians, and referral sources
• Participate in daily stand-up and stand-down meetings, case conferences, and required trainings
• Communicate patient condition changes and care needs promptly to the interdisciplinary team
• Accurately document all patient care in the electronic medical record (Homecare Homebase)
• Support continuity of care and uphold agency and regulatory standards

Required Qualifications:

• Active Registered Nurse (RN) license
• Home health experience required
• Homecare Homebase (HCHB) experience preferred
• Ability to work independently in a field-based, travel role
• Strong assessment, communication, and organizational skills

Preferred Qualifications:

• Experience with wound care and wound vacs
• Strong collaboration and teamwork skills
• Flexibility and comfort working in both home health and hospice environments
