Work Environment

This role is primarily onsite, providing home care services directly to patients within the Woodbridge area. Working closely within the community, you will create a meaningful connection with your patients, addressing their unique needs and circumstances in a familiar environment. Key Responsibilities

• Conduct thorough evaluations of patients by reviewing their medical history, performing assessments, and documenting results to create customized treatment plans.
• Engage with patients and their families to explain evaluation findings, treatment options, and collaborate on goal setting.
• Implement evidence-based treatment protocols tailored to each patient's needs, while adjusting these protocols based on ongoing assessments and changes in the patient's condition.
• Coordinate and consult with multidisciplinary healthcare professionals to provide comprehensive care.
• Maintain up-to-date and accurate documentation of patient care activities, including initial evaluations, daily encounter notes, progress summaries, and discharge evaluations.
• Participate in discharge planning and ensure thorough follow-up care as required.
• Maintain a well-organized schedule, ensuring punctuality for patient appointments and participation in marketing and outreach initiatives.
• Contribute to departmental goals and compliance with local and federal regulations, with an emphasis on continuous quality improvement.

Patient Population / Client Focus

Your work will focus on a diverse population needing physical therapy services, ranging from post-operative recovery to chronic pain management. You will encounter patients from various backgrounds, tailoring your approach to address their individual circumstances, preferences, and health goals, therefore playing a vital role in their recovery journey.

Qualifications

• Completion of a Bachelor's Degree in Physical Therapy from an accredited program.
• A valid state license to practice as a Physical Therapist in Virginia.
• Certification in BLS for Healthcare Providers from the American Heart Association is mandatory.
• Strong interpersonal skills to engage effectively with patients, families, and interdisciplinary teams.
