Palliative Care, LPNThe Palliative Care, LPN is responsible for the evidence-based care of a group of Veterans as follows:The duties and assignments of this position include interviewing Veterans, performing a general systems appraisal of the patient's condition, collecting objective and subjective data. Duties also include collaboration with the RN in developing, selecting the appropriate sequence of procedures, and implementing a plan of care individualized to carry out the nursing needs of the patient.Contributes to the interdisciplinary team (IDT) in the development, implementation, and evaluation of the Veteran's outpatient treatment and discharge plan if applicable. Collaborates with members of other services regarding changes in Veteran's condition, laboratory results, responses to treatment plan, and other pertinent parameters of care.Promotes Veteran/family/caregiver/staff relationship by utilizing appropriate communication skills.Prepares and administers commonly prescribed medications (oral, topical, subcutaneous, intramuscular and/or intravenous) as permitted by approved local facility policies and procedures. Observes patient's response to medication administered and reports any noted change in patient's condition to RN or MD/DO.Performs support duties for common diagnostic tests and/or practices or procedures, which include preparing the patient, assisting in the diagnostic examination, preparing specimens, preparing, and passing instruments, and monitoring the patient's condition before, during, and after the procedure. Works with the RN and/or MD/DO to appropriately orient/train less experienced LPNs/licensed vocational nurses (LVNs) or nursing assistants (NAs), or health technicians (HTs) in relation to acceptable standards of practice in promoting optimal patient care delivery.Recognizes and appropriately responds to breakage/malfunction or loss of equipment, safety hazards, and supply deficiencies, promptly reporting to appropriate personnel for corrective action.Recognizes the need for and institutes emergency measures when indicated, promptly seeks the assistance of the RN, or MD/DO and assists in resuscitation procedures in cardiac and/or pulmonary arrest.Educates patients and significant others in relation to common disease processes, medication, and/or prescribed treatment regimes. Documents patient education and patient's response.Safeguards the patient's right to privacy by protecting information of a confidential nature. Congruent to Health Insurance Portability and Accountability Act of 1966, maintains information security management.Provides one-to-one interactions to explore feelings and behavior and promote interpersonal relationships, communication, and socialization skills in accordance with the patient's plan of care. Participates in unit quality management (QM) programs. Follows established safety practices and, reporting to the supervisor or RN, unsafe conditions, unsafe practices, or any occupational illness or injury. Adheres to all safety and health standards, regulations, and work practices.Additional duties as assigned.
