Description

• Assess patients in both inpatient and outpatient environments and craft personalized treatment plans
• Provide therapeutic interventions aimed at enhancing mobility, strength, balance, and overall function
• Collaborate effectively with healthcare professionals, including nurses, physicians, case managers, and rehabilitation specialists to ensure optimal patient outcomes
• Guide patients and their families in understanding therapeutic objectives, discharge strategies, and at-home exercises
• Document evaluations, treatments, and progress diligently as per facility and regulatory standards
• Engage in care team rounds and interdisciplinary conferences to guarantee seamless care delivery

Education

• Doctorate in Physical Therapy (DPT) or equivalent degree from a recognized institution

Certifications

• Current state licensure as a DPT / Physical Therapist
• CPR certification is preferred
• Specialized certifications (such as neurological or geriatric) are beneficial, though not mandatory

Skills

• Previous clinical experience is strongly preferred
