Key Responsibilities Deliver occupational therapy treatments as outlined in care plans Assist residents with activities to improve ADLs, mobility, and coordination Monitor patient progress and report updates to the Occupational Therapist Maintain accurate and timely documentation Ensure a safe and supportive therapy environment Collaborate with the interdisciplinary care team Qualifications Associate degree from an accredited Occupational Therapy Assistant program Active, unencumbered COTA license in South Carolina Experience in a long‑term care or rehab setting preferred Familiarity with PointClickCare (PCC) and Casamba preferred Strong communication and interpersonal skills Ability to work independently and as part of a team Why Join Chandler Creek Post Acute?
