Overview Project Manager, Health Systems North — Full-Time Professional, New York, NY, US. This is a grant-funded position ending April 30, 2031. See healthsolutions.org for more information. Program Description NYC Treat to Quit is a program of Public Health Solutions and is funded primarily by the New York State Department of Health. The mission is to advance health equity by strengthening medical and behavioral health systems, expanding access to evidence-based tobacco dependence treatment, and empowering communities disproportionately impacted by tobacco-related harm. Current objectives under NYSDOH contract include: (1) reducing tobacco-related health disparities in the catchment area; (2) collaborating with medical and behavioral health systems to adopt the Clinical Practice Guideline for Treating Tobacco Use and Dependence; (3) enabling grantees to be current with medication treatment protocols for tobacco use disorder; (4) advocating for local and state laws and policies to reduce tobacco exposure; (5) supporting statewide data collection and dissemination of resources to raise public awareness and action on tobacco use and cessation. Summary of Position Project Manager, Health Systems will identify and lead the implementation of health systems engagement strategies with hospitals, clinics, behavioral health centers and FQHCs in the Bronx, Manhattan and Staten Island. The role supports the integration of evidence-based cessation interventions, staff training, EHR prompts, and continuous quality improvement to strengthen clinical systems change. The Project Manager will work with the NYC Treat to Quit team and report to the Senior Health Systems Manager under the Program Director. This is a grant-funded position ending April 30, 2031. Responsibilities Understand and implement the NYS Department of Health Tobacco Control Program’s strategic plan and support the development of PHS’ tobacco control strategic plan. Provide education, technical assistance, and implementation support to Community Health Centers, FQHCs, LGBTQIA+ Health & Human Services Network, and other providers; including systems serving people with serious mental illness and/or substance use disorders. Collaborate with NYC Treat to Quit staff to achieve region-specific work plan deliverables. Work with clinical staff to implement and enhance tobacco cessation policies and protocols, including the Ask-Advise-Refer model and other evidence-based interventions. Coordinate with clinical leadership, quality improvement teams and IT/EHR staff to optimize prompts, decision support tools and referral pathways. Train and coach healthcare providers on tobacco screening, brief intervention, and cessation counseling. Support hospitals, FQHCs, behavioral health systems and coalitions to implement tobacco-free policies and strengthen referral pathways. Promote long-term integration and sustainability of tobacco dependence treatment in clinical workflows and organizational policies. Lead outreach, training, engagement and education activities with healthcare systems and stakeholders to implement evidence-based tobacco treatment and systems change initiatives. Coordinate trainings, meetings, and quality improvement sessions; monitor progress and identify barriers, proposing CQI strategies. Track cessation performance data to improve implementation and address disparities. Develop educational materials and communications to support program goals for clinical leadership, administrators, and policymakers. Ensure timely reporting of outcomes and activities; maintain documentation in required formats; collect and synthesize program data for monitoring and quality improvement. Participate in state meetings and workgroups; contribute to a robust health systems engagement strategy aligned with organizational goals. Qualifications and Experience Bachelor’s degree or higher in a related field. At least 3 years of experience integrating public health in healthcare and behavioral health systems. Experience with policy and community health systems interventions, health systems initiatives and partnership building. Knowledge of tobacco cessation best practices and treatment approaches; pluses for emerging issues. Excellent oral and written communication and interpersonal skills; ability to engage with clinical leadership and community stakeholders. Experience developing and delivering structured training rooted in adult learning principles. Ability to engage with partners during evenings/weekends and maintain a flexible schedule. Strong organizational skills; ability to coordinate multiple tasks with competing priorities; able to work independently and as part of a team. Familiarity with New York City communities strongly preferred. Willingness and ability to travel within NYC and across NYS; residency in the tri-state area by start date is required. Must reside in the tri-state area (NY, NJ, CT) by confirmed start date. Reports To Director of Tobacco Control Alliance and Senior Manager Health Systems .
