Social Worker/Case Manager - CPS

Juneau County (WI)

POSITION SUMMARY

This position is responsible for providing needs assessments, crisis intervention, case planning, advocacy, and development of community resources; providing services to both children and adults with varying abilities and functioning levels. The position of Case Manager will be assigned a designated Division within the Human Services Department.

ESSENTIAL FUNCTIONS

The following duties are normal for this position. However, they are not to be construed as exclusive or all‑inclusive. To perform this job successfully, an individual must be able to perform each duty satisfactorily. Other duties may also be required or assigned. Provides assessment, treatment planning, and case service facilitation/case management services to assigned clientele per the rules, laws, and regulations of assigned program area. Works to achieve involvement and participation of the service recipient, their family, and a natural support system in resolving problems. Works with service recipients to assess and develop goal‑oriented, time‑limited service plans to address identified needs. Works to protect children, juveniles, elderly, and at‑risk adults; strives to maintain existing family units and individual autonomy in the community; or helps prevent out‑of‑home placement. Assesses and evaluates prospective service recipient’s immediate situation, determines appropriate response, and assures that response is made. Assesses and intervenes as necessary in crisis situations. Refers to and involves other community resources as needed. Works to achieve active involvement and meaningful participation with schools, courts, health care providers, and other appropriate community resources in resolving problems. Advises service recipients when services are either not available or inappropriate. Explains the consequences of non‑compliance with court orders to service recipients who are involuntarily referred, monitors utilization of services, and informs the court of consumer's adherence to court orders as directed. Coordinates access to services, monitors cases to ensure services are provided in an appropriate and timely manner, reviews progress, and terminates services when goals are attained. Educates service recipients about available community resources and how to independently gain access to needed services. Advocates and works to coordinate services and support service recipients in achieving treatment objectives without duplication of effort and with maximum effective use of staff time and available resources. Maintains a timely and complete record for all assigned cases, including within the agency’s Electronic Health Record (EHR) system and/or the State’s eWiSacwis/Adult Protective Services/Elder Abuse reporting systems; completes required written assessments, reports, and other related paperwork/documentation as necessary. Consults with supervisors, interdisciplinary teams, and legal counsel on appropriate case‑specific matters, as necessary. May facilitate treatment and/or other related, indicated services with groups of service recipients. May be designated to speak to the community and other organizations to provide public education. Develops and maintains professional knowledge and skills to enhance the programs and services provided. Other duties as assigned.

EXPECTATIONS

Interprets and applies Federal, State, municipal, and Department rules, regulations, and policies. Attends work regularly and is punctual. Behaves professionally and presents an appropriate standard of grooming, hygiene, and dress that aligns with health and safety standards. Conducts self in such a way as to reflect positively on the County. Fosters positive working relationships and accepts new responsibilities. Learns and understands the safe practices for the job being performed and complies with the safe work practices and personal protective requirements for the job.

REQUIRED KNOWLEDGE, SKILLS, AND ABILITIES

Knowledge of laws and regulations that affect the delivery of services provided by the Department. Knowledge of community resources and alternative care resources. Working knowledge of mental health and AODA assessment, diagnosis, and treatment. Working knowledge of client rights, confidentiality, standards of practice, and ethics in mental health/AODA field. Working knowledge of applicable Wisconsin State Administrative Codes, including but not limited to, Chapters 34, 35, 36, 48, 51, 55, 63 and 75; and Medicaid Waiver Manual (CLTS). Knowledge of human development and behavior, family and group dynamics, and mental, physical, and social dysfunctions. Working knowledge of family‑based services philosophy. Knowledge of administrative policies and procedures of the County. Knowledge of county, state, and federal programs pertaining to regulations and mandates under which the department functions. Ability to utilize various computer software programs that are department‑ and program‑specific. Ability to maintain accurate records of assigned activities. Skill in time management; prioritizing duties to accomplish a high volume of quality work product requiring minimal supervision. Skill in analyzing complex administrative information and issues, defining problems and evaluating alternatives, and recommending methods, procedures, and techniques for resolution of issues. Ability to define problems, exercise sound judgment, and address a variety of situations successfully. Knowledge and ability to follow the policies and practices contained in the County Personnel Handbook. Ability to work the allocated hours of the position and respond after hours as assigned.

MINIMUM REQUIRED QUALIFICATIONS

Bachelor’s degree from an accredited college or university in a Human Services‑related field. Must successfully pass caregiver and criminal background check.

Work has standard vision requirements. Verbal communication is required for expressing or exchanging ideas by means of the spoken word. Hearing is required to perceive information at normal spoken word levels. Work requires preparing and analyzing written or computer data and observing general surroundings and activities. Ability to recognize occupational hazards in work activities and take safety precautions. Work may involve driving in inclement weather conditions. Work is performed primarily in the community, in clients’ homes, and in an office setting, although work may be performed in a secured inmate setting.
