About This Role The Triage Registered Nurse utilizes the nursing process to evaluate patient needs over the telephone, providing guidance and education while adhering to organizational policies. This role is ideal for nurses who excel in communication and patient education. Highlights Location: Melville, NY Schedule: Full-Time, Day shift What You'll Do Perform outbound calls to specific patient populations. Re-enforce discharge plans and medication management. Receive, triage, and respond to telephone calls from patients requiring medical guidance. Provide patients with clear instructions and education utilizing appropriate guidelines. Direct patients to the proper locus of care based on nursing assessment outcomes. Document calls according to established guidelines. Communicate with physicians, RNs, or other healthcare providers regarding patient status. Collaborate with physicians when patient intervention or follow-up is required. Participate in education and in-service programs. Perform related duties as required. Requirements Education: Graduate from an accredited School of Nursing; Bachelor’s Degree in Nursing preferred. Must be enrolled in an accredited BSN program within two (2) years and obtain a BSN Degree within five (5) years of job entry date.
