Job Title: Physical Therapist (PT) – Long-Term Care / Skilled Nursing Facility (LTC/SNF)
Location: Seattle, WA

Duration - 13 Weeks

Job Overview

We are seeking an experienced Physical Therapist (PT) for a Long-Term Care/Skilled Nursing Facility setting. This position offers the opportunity to work with residents requiring rehabilitation services focused on improving mobility, function, safety, and overall quality of life.

Schedule

8-Hour Day Shifts
No Weekends
No On-Call

Requirements

Active Physical Therapist License (pending license accepted)
BCLS Certification required
Minimum 1 year of Skilled Nursing Facility (SNF) and/or Long-Term Care (LTC) experience required
First-time travelers not accepted
Reliable transportation required

Responsibilities

Evaluate residents and develop individualized therapy treatment plans.
Provide therapeutic interventions to improve strength, balance, mobility, endurance, and functional independence.
Perform ongoing assessments and adjust treatment plans based on patient progress.
Collaborate with physicians, nursing staff, occupational therapists, speech therapists, and interdisciplinary teams.
Educate patients, families, and caregivers regarding rehabilitation goals and home exercise programs.
Complete timely and accurate clinical documentation.
Ensure compliance with facility, state, and federal rehabilitation standards.
