Intermountain Health is seeking an RN Care Manager to coordinate patient care across settings in our mission-driven, team-based environment. The RN Care Manager will complete comprehensive assessments, develop and implement individualized care plans, facilitate safe discharges, and ensure appropriate use of resources. In this role, you will collaborate closely with physicians, social workers, and allied health professionals to improve quality, safety, and patient experience.
