Hospice RN Case Manager — Flexible, Home-Based Care

St. Mary’s Home Care and Hospice is seeking a Part-time Contingent Hospice RN Case Manager (PRN) to provide professional nursing care to hospice clients in home settings. The RN will assess, plan and supervise patient care, working with physicians and an interdisciplinary team to manage symptoms and support families. The role requires PA RN licensure and at least 2 years of nursing experience in acute, home care or ER settings, with EPIC experience preferred. #J-18808-Ljbffr IHA
