Schedule

Night shifts from 23:00 to 07:00, working on a 5x8-hour schedule.

Job Description

We are seeking a dedicated Licensed Practical Nurse (LPN) for a Long-Term Care facility with an average daily census of 21 patients. You will work in a setting accommodating 23 beds, focusing on providing exceptional patient care in collaboration with an interdisciplinary team.

Responsibilities

• Manage patient care, including medication administration and treatment compliance.
• Conduct patient assessments and monitor vital signs.
• Assist with daily living activities and provide emotional support to patients and their families.
• Collaborate with a multi-disciplinary team to develop and implement care plans.
• Ensure adherence to safety and regulatory standards.

Required Experience / Certifications / Licensure

• A minimum of 1 year of recent LPN experience is required.
• Previous charge experience is required.
• BLS certification is required.
• Must possess a valid NY state license to practice as an LPN.
• Experience with the EPIC charting system is a hard requirement.
