Key Responsibilities\n

• Conduct thorough assessments of clients' social, emotional, and environmental challenges to develop a holistic understanding of their needs.

• Create tailored service plans that outline specific goals, interventions, and methods for tracking client progress.

• Provide compassionate counseling and individualized support to individuals and families grappling with chronic illness, grief, and other end-of-life issues.

• Collaborate with clients to connect them with appropriate community resources to facilitate access to care, housing, or financial assistance.

• Implement crisis intervention strategies, conduct risk assessments, and create safety plans as required based on client circumstances.

• Engage with community agencies and organizations to streamline access to services for clients, advocating on their behalf as needed.

• Participate in regular case conferences and team meetings to share insights, strategies, and support with colleagues.

• Participate in community outreach initiatives to raise awareness of the hospice services offered by Legacy Healing Center.

Patient Population / Client Focus\n

In this role, you will work primarily with individuals and families experiencing terminal illness and the accompanying emotional and psychological challenges. Your expertise will help ensure that clients receive holistic support tailored to their specific needs, aligning with our mission to address the root causes of distress and foster empowerment.

Qualifications\n

• A Master's degree in Social Work from an accredited institution is required.

• Current state licensure as a Licensed Independent Clinical Social Worker (LICSW).

Compensation and Benefits\n
