Hospice Inpatient Clinical Coordinator (RN)

Compensation

Salary Range: $115,000 - $125,000 per year

Position Summary
As a Hospice Inpatient Clinical Coordinator (RN), you will support hospice admissions and inpatient care delivery within an innovative hospice and palliative care program. This role combines clinical expertise with care coordination and relationship management, ensuring high-quality, patient-centered care for individuals with life-limiting illnesses. You will collaborate closely with interdisciplinary teams, manage hospice admissions, and ensure compliance with regulatory standards in a fast-paced inpatient setting.

Responsibilities

• Schedule / Shift

• Shift details: Days (flexible, including weekends as needed)
• Hours per week: 40

• Key Duties:

• Conduct comprehensive clinical assessments and facilitate hospice admissions
• Build and maintain referral relationships with hospital partners
• Coordinate symptom management and care plans for General Inpatient (GIP) hospice patients
• Collaborate with interdisciplinary teams and participate in IDT meetings and care huddles
• Develop and manage individualized nursing care plans
• Ensure accurate, timely, and compliant clinical documentation
• Provide hospice education and clinical support to hospital staff

Requirements

• Education

• Required: Graduate of an accredited School of Nursing
• Preferred: Bachelor of Science in Nursing (BSN)

• Licenses & Certifications

• Required: Active New York State Registered Nurse (RN) license; CPR-BLS certification

• Strong clinical assessment and hospice care coordination skills
• Excellent communication and interdisciplinary collaboration abilities
• Advanced critical thinking and problem-solving skills
• Ability to manage patient care in a fast-paced inpatient environment
