You will engage with patients and their families through virtual platforms, utilizing teletherapy as a primary mode of service delivery. Key Responsibilities\n

• Utilize your education, training, and therapeutic approach to conduct thorough psychosocial assessments and formulate independent diagnoses for patients.

• Collaborate with patients to create individualized, patient-centered treatment plans that reflect their specific preferences, strengths, and goals.

• Implement evidence-based therapeutic practices, such as solution-focused therapy, behavioral activation, and motivational interviewing to facilitate behavioral change.

• Work closely with a network of healthcare professionals, including primary care providers and psychologists, to foster an integrated approach to client care.

• Consistently monitor treatment response through telehealth sessions, tracking patient progress towards established objectives and modifying treatment plans as necessary.

• Support medication management practices, collaborating with medical providers to monitor treatment adherence and assess any side effects or necessary dose adjustments.

This role will primarily focus on patients receiving hospice care, often dealing with terminal illnesses and significant psychosocial stressors. Qualifications\n

Compensation and Benefits\n

The compensation for this role is competitive, with an annual salary ranging from $80,000 to $100,000, commensurate with experience.
