If you're a dedicated nurse who thrives on autonomy and patient advocacy, we want to hear from you!ResponsibilitiesConduct comprehensive patient assessments and develop individualized care plans for home health clientsCoordinate and manage all aspects of patient care, including scheduling, referrals, and interdisciplinary team communicationPerform skilled nursing visits, monitor patient progress, and adjust care plans as clinically indicatedEducate patients and family members on diagnoses, medications, and self-care techniquesCollaborate with physicians, therapists, and other healthcare professionals to ensure continuity of careMaintain accurate and timely clinical documentation in compliance with regulatory and agency standardsEnsure patient safety and advocate for optimal health outcomes throughout the care continuumRequirementsActive RN license in the state of Colorado (or compact license eligibility)Previous home health, case management, or community health nursing experience preferredStrong clinical assessment and critical thinking skillsExcellent communication, organization, and time management abilitiesProficiency with electronic health records (EHR) and clinical documentationValid driver's license, reliable transportation, and willingness to travel within Northern CO service areaCompassionate, patient-centered approach with a commitment to quality careComForCare Home Health Care - Northern CO is a trusted provider of in-home health services dedicated to helping clients live their best lives in the comfort of their own homes.
