Registered Nurse Case Manager – Home CareComForCare is hiring an experienced RN Case Manager in Fort Collins, Colorado and surrounding areas. This place really enhances one's customer service skills and lets you build meaningful relationships with clients." — ComForCare RNWhat You'll Do as an RN Case ManagerClient intake and assessments: Complete initial and ongoing in-home nursing assessments of each client's physical, cognitive, psychosocial and home safety needsCare plan development: Create and update individualized plans of care and make sure caregivers understand and follow themCaseload management: Oversee a caseload of home care clients, tracking changes in condition and adjusting services as needs changeCaregiver supervision and training: Conduct supervisory visits and complete competency checksCare coordination: Serve as the main clinical contact for families, physicians, hospitals and community partners; make referrals when neededDocumentation and compliance: Keep accurate, timely client records and follow Colorado home care regulations, infection control and HIPAA guidelinesOn-call support: Help resolve client and caregiver concerns quickly and professionallyQualificationsActive, unencumbered Registered Nurse (RN) license in ColoradoAt least 1 year of home care or home health nursing experience (required)Case management, care coordination or caregiver supervision experience (preferred)Current BLS/CPR certificationValid driver's license and reliable transportationOasis DocumentationStrong clinical judgment, organization and communication skills, plus a genuine passion for client-centered careReady to Lead Care Your Way?
