Field-Based Care Coach: LTSS & Medicaid Support
Humana
Humana is seeking LTSS Care Coordinators (Care Coach 1) in Illinois to assess members’ needs and guide families toward appropriate resources. The role requires extensive field visits (75–90% travel) and coordination with care teams to ensure services align with each member’s goals. You will work in a field setting, travel within the region, and participate in Humana’s TB and driver safety programs. Monday–Friday schedule with 40 hours weekly. #J-18808-Ljbffr Humana
