Patient Care Coordinator

Associate Patient Care CoordinatorThis position is responsible for accomplishing various tasks that contribute to the organization's success and patient satisfaction. The team the candidate will be working with typically consists of Providers, Medical Assistants, Nurses (RN/LPN), PSA, echo techs averaging 21 members daily. The team culture is characterized by high morals and strong interpersonal skills, which are essential for success in a fast-paced environment. The candidate will be responsible for executing day-to-day operations, ensuring compliance with regulations, and providing coverage across other sites as needed. The team's diversity, including various ethnic backgrounds, age groups, and professional experiences, fosters a rich exchange of ideas and enhances creativity and problem-solving abilities.Primary Responsibilities:Obtain accurate and updated patient information, such as name, address, insurance informationPerform insurance verification on the date of serviceObtain patient signatures for required documentsFile, Fax and maintain medical recordsConfirms and schedule appointmentsAnswering incoming and outgoing telephone calls promptly and courteouslyFollow the Payment Security policy and procedures according to Optum Medical guidelinesAbility to maintain a work queue list- complete any pending actions ie referrals, scheduling appointmentsCheck In and Check Out patientsAbility to maintain and work the front desk in-basket pool.Perform referral documentation promptlyPerforms certain follow-up services for patients in a prompt and courteous manner, such as scheduling specialist appointmentsAdhere to the standards identified via Sparq regarding Optum Employee PoliciesWork cohesively with fellow employees to achieve specific team goalsComfortable working in high pace environmentAssure the continuity of care through scheduling and tracking systemsProvide effective communication to patient / family team members and other health care professionals as evidenced by documentation, case conferences, communication notes, and evaluationsComply with administrative policies to ensure quality of careDemonstrate precision and efficiency in scanning documents and monitoring the fax server, retrieving and / or scanning documents and assigning to the appropriate electronic chartAll employees are expected to keep abreast on current medical requirements relevant to their position, which includes maintaining patient confidentiality and abiding by all HIPAA and OSHA requirementsCrosstrain and assist in other locations if neededPerforms other duties as assigned
